Step 1: Read CYO Philosophy, Code of Conduct, and Rules sections Step 2: Confirm Eligibility 1. Residence Player must either reside within Saint Monica’s Parish boundaries or be enrolled in Saint Monica’s Faith Formation classes for the 2009/2010 year. Approximately, 90% of Saint Monica’s Parish boundaries are within the 94556 Zip code. 2. Age a. Maximum age per grade as of Sept 1, 2009 in order to be eligible 3rd Grade – 9 4th Grade – 10 5th Grade – 11 6th Grade – 12 7th Grade – 13 8th Grade – 14 b. All New Players must provide a copy of the Player’s birth certificate at the evaluations. Step 3: Determine if you need to order a uniform during the registration process? a. New player or players for did not participate last season must order a uniform. b. Returning players from last season do not need to order a uniform unless they lost the uniform or grew out of it. c. Uniform cost is $50 Step 4: Complete Registration Online at Active.com (09/15/09 Closed) or by completing a registration packet available at St. Monica parish office. · Registration fee must be paid at the time of registration o $185 per player for Parishioner o $210 per player for Non-Parishioners Step 5: Participate in player evaluation scheduled for the weekend of Sept. 19th and 20th a. Following Forms must be submitted at Evaluations o Birth Certificate (new players only) b.
Sample Step 6: Teams are formed, volunteer coaches are selected and announced by CYO Athletic Director Frequently Asked Questions Saint Monica’s CYO Basketball is a 100% volunteer run program. Parents of participates are expected to volunteer in order to enable this program to exist. Participation is open to anyone residing within the Saint Monica parish boundaries or enrolled in Saint Monica’s Faith Formation classes for the 2009/2010 year that meet the age requirement. Can I volunteer to
coach my son or daughters team? Yes, volunteer coaches are a critical part in forming teams. What happens if we
don't have a volunteer coach? Saint Monica’s CYO program depends on volunteers to be successful and registration fees are not sufficient to pay for a non-volunteer coach. Team parents may submit a request to the Athletic Director for approval to pay an outside coach for the season. Team parents are solely responsible for recruiting and any compensation required for an outside coach. Good coach sources include relatives, local high school, local colleges and prior CYO players and coaches. Any outside coach must complete the same requirements as a volunteer coach. Under what conditions can I receive a refund? Refunds will be granted if requested prior to evaluations (last to request refunds September 17, 2009). Refund must requested by email to stmocyo@comcast.net. If registration was processed via our online service, refunds are subject to a 10% processing fee which is not refundable. Can I still sign up
after Sept. 14? After Sept. 14 you may request to be placed on a wait list
by (1) sending a request by email to stmocyo@comcast.net
and (2) completing a registration packet available in the Parish office at
St. Monica. All players
completing registration prior to Sept. 15th must first be placed on
a team before a wait list player may be placed on a team. If an opening is available on a team,
